Issue:

On August 19, 2014, during a scheduled Town Hall Meeting held by the Director for Veterans, a Veteran announced that when he requested copies of his medical record, he also received about 250 pages of another Veteran's medical records. Media and Representatives from various local Congressional offices were also in attendance of this town hall and would have overheard the statements by this Veteran. On August 20, 2014, the PO was notified of this reported incident and contacted Veteran A. The Veteran stated that he requested and received a copy of his medical records from the Nashville Campus Release of Information Office back in March 2012. He stated about a month ago he finally started reviewing those records and discovered he also received some medical records belonging to Veteran B. Veteran A stated that he has been attempting to contact Veteran B to provide those records back to him, but has been uable to make contact. Veteran A refuses to return the records of Veteran B to this facility, and only agrees to provide directly to Veteran B, but did state he would provide them if a meeting could be arranged between Veteran A and Veteran B.

Outcome:

08/20/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
