Issue:
Based on interview and record review, the hospital failed to ensure that Patient 1's medical information was kept confidential from anyone not authorized access.Findings:An interview with the health information management lead (HIML) was conducted on 5/10/12 at 11:20 A.M. The HIML stated that a health information management staff was processing several discharge summary requests. The HIML stated that the staff mistakenly faxed Patient 1's discharge summary to the wrong requesting entity. The discharge summary that was inadvertently sent to the wrong recepient indicated Patient 1's name, date of birth, medical record number, diagnoses, radiology exams and the results.
