Issue:
A consult written for Veteran A was erroneously documented in Veteran B's chart. Veteran B received a copy of the information and reported the error to VA. The identifiers for Veteran A that were included in the consult are Veteran A's first and last name and diagnoses. Veteran B has agreed to return the document to VA custody.
Outcome:
05/02/14: The Incident Resolution Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
