Issue:
The spouse of a Birmingham VA Medical Center (BVAMC) patient presented to the Patient Services Assistant (PSA) documents that spouse requested be delivered to a BVAMC Resident. The PSA advised the spouse that the Resident was gone for the evening, but that PSA would get the documents to said Resident. The spouse handed the documents to the PSA who noticed the documents were physician's rounding sheets which contained PHI of 9 different BVAMC patients. The PSA put the documents in an envelope and delivered them to the team room and advised the residents in the team room of the incident. PSA then notified Nurse Manager. Nurse Manager attempted to locate the spouse, but was unsuccessful.
Outcome:
11/25/13: Nine (9) Patients will be sent letters offering credit protection services due to full SSN being disclosed.
