Issue:

VA police contacted the Privacy Officer (PO) to let him know they forwarded him a patient's Veteran Identification Card (VIC) they had received in the mail. The PO contacted Veteran A, the owner of said card, and he stated that at the blood lab they told him they had given someone else his card. The PO then contacted Veteran B, who had received the wrong card, he stated when he got home he realized that the card was not his and noticed it had Veteran As Name, Full SSN and DOB, so he immediately sent it back. The blood lab told Veteran B when he called that his card is waiting for him at the main check-in desk. The PO will bring Veteran As VIC there as well so he can pick up at his next appointment. Update: 05/10/11:Veteran A will be sent a letter offering credit protection services, due to full name, SSN and DOB being exposed.

Outcome:

Employee had to retake privacy training. Credit Monitoring letter sent 5/16/11
