Issue:

The Administrative Officer of the Day (AOD) found an appointment list containing 31 patient names, cell phone, diagnosis and date/time of appointment in the Atrium of the Medical Center. It appears an employee left it on a table. The Weekend AOD secured the document and brought it to the Privacy Officer this morning. The table in the atrium is an open area. There doesn't appear to be any missing pages.

Outcome:

07/07/14: The report was printed on Friday, so it could have been possibly out in the atrium from Friday 8 AM until Saturday Morning 8:30 AM when the employee saw and secured it. The Privacy Officer will be interviewing two clerks that sit in the Atrium as part of the Info Desk (about 15 feet away from where the list was found) to see if they saw anyone looking through the list. Additionally, the Privacy Office will be asking Police Service if there are any cameras in the area that can be reviewed to see if it can be determined if the list was accessed by any unauthorized person prior to being secured. 07/28/14: The Incident Resolution Service Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
