Issue:
The Assistant Chief of Staff (ACOS) of Ambulatory Care contacted the Privacy Officer (PO) to report that the Fecal Occult Blood Test (FOBT) cards of a Veteran had been delivered to an incorrect address. The intended recipient reported this incident. The USPS incorrectly delivered the information to a neighbor's address. The patient also reported that her identity had been previously stolen in the past. The PO will conduct a fact-finding to identify the process failure that caused this error to occur. Update: 04/01/11:This was a USPS error but VA will offer credit protection since the full SSN was exposed.
Outcome:
This process has been changed where the full SSN is no longer printed on these labels.
