Issue:

A Release of Information (ROI) Clerk, while processing a request from Veteran A for a copy of his DD-214, accidentally handed him the DD-214 of Veteran B. The DD-214 was taken to the office of the Veteran Service Officer (VSO) for the purpose of filing a claim. The Administrative Assistant who worksin the VSO office discovered the error and immediately notified the ROI Dept. This clerk also shredded the incorrect document. Veteran A then returned to the Release of Information Section, where he received the correct document. Update: 04/02/12:Veteran B will be sent a letter offering credit protection services, as his full SSN was disclosed.

Outcome:

P.O. MET WITH EMPLOYEE TO REMND HIM OF THE NEED TO PROCESS ROI REQUESTS CAREFULLY TO ENSURE THAT NO PRIVACY BREACH OCCURS.
