Issue:
The Supervisor, Release of Information (ROI), notified the Privacy Officer (PO), Nashville Campus, of the following incident: A Clerk in the Release of Information (ROI), Nashville Campus, inadvertently sent copies of medical records to a Paralegal Agency on the wrong Veteran. The request received was for Veteran A but the Clerk sent records belonging to Veteran B. The Clerk learned of the error when he was contacted by the Paralegal Agency. The Supervisor, ROI, will make arrangements to retrieve Veteran Bs documents. Veteran Bs personal identifiable information includes: full name, full social security number, date of birth, contact information, progress notes, medications, EKG, electroencephalogram, and vitals.
Outcome:
07/22/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services due to full SSN being disclosed.
