Issue:
Patient A received the correct medical supply from Medline Industries but noticed that when their medical supply package was delivered, the medical supply content description on the outside of box was unmasked or not "blacked out." Patient A's type of medical supply was potentially compromised. Patient A reported the incident to the Columbia VA Medical Center (site 589A4). Leavenworth Consolidated Mail Outpatient Pharmacy (CMOP) investigation concludes that this was a Medline packing error. The packing error has been reported to Medline for investigation and corrective action.
Outcome:
11/24/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
