Report ID: SPE000000060678, U.S. Department of Veterans Affairs
Reported Entity: VISN 19 Salt Lake City, UT
Issue:
Fifteen (15) Veterans Identity Cards (VIC) were found on the flight line at the airport. The original mailing box had been crushed and the contents were laying out on the tarmac. The address and the return address on the box were unreadable so the employee threw the original mailing box in the trash. The airport employee gathered up all of the letters that they could find and returned them to the facility. The letters looked like they had been laying in water and were damaged, but the VIC cards were intact. Update: 04/09/11:The Veteran's full name is viewable on the card and the full SSN is in the bar code which is readable with most scanners. The box fell off the cart that the airport uses to transport cargo from the airplanes to the terminal. It was then run over and destroyed, at which time, the contents were spilled out onto the tarmac. Fifteen Veterans will receive a letter offering credit protection services.
Outcome:
16 Credit Monitoring letters were sent out on 4/13/11 - A redacted copy of 1 of these letters was sent to the VAIRCT mailbox so this ticket could be closed.Informed the HEC of this incident.
