Issue:

Patient A came in to see his physician and in the process was given a copy of his lab results. When he got home and looked at them, Patient B's information was included also. His daughter called and was upset because her father thought he had a serious condition and then to find out the labs were not his. As of right now Patient A still have Patient B's information. According to Patient A he will bring them in on 08/13/12. Patient B's information at risk includes his name, address, date of birth, full SSN and lab results. Update: 08/13/12:Veteran B will receive a letter offering credit protection services.

Outcome:

Staff was reminded not to allow patients to remove any documents from their desk. All documents are to be double check before giving it to any patient for accuracy.
