Issue:

Patient A, brought the Patient Advocate, 128 pages of another Patient's records he received from Release of Information (ROI). His concern is that his records may have gone to the other patient. Patient A returned the records to Patient Advocate. Patient Advocate notified the VA Employee responsible for the mishandling of the patient records. The employee then printed off the correct patient's records, and confirmed the other patient, did not receive Patient A's records because that request was not completed yet. Update: 05/18/12:Patient B will be sent a letter offering credit protection services.

Outcome:

Employee was educated regarding responsibility to ensure disclosing information to the appropriate patient/requestor.
