Issue:

On 08/01/11, a Veteran/employee requested a Sensitive Record Access Rreport because he suspected one of his co-workers might be inappropriately accessing his medical record. A Sensitive Record Access Report showed that the employee in question accessed his record on three occasions. The most recent was a year ago on 07/27/10 and twice on 11/06/09. Update: 08/01/11:The employee will be sent a letter offering credit protection services, since his full SSN and date of birth were disclosed.NOTE: There were a total of 17 Mis-Handling incidents this reporting period. Because of repetition, the other 16 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

Employee's access to CPRS has been removed and disciplinary action to be taken.
