Issue:

Veteran came to the MEDVA MC on 04/29/15 to complain about services he received in the ER on 04/08/15. During the conversation he informed the Health Systems Specialist (HSS) that he returned to the ER on 04/11/15 and while there requested a copy of his 04/08/15 visit. The ER clerk gave him copies of another Veteran's ER visit. He did not state when he discovered he had the wrong records. He did not bring the paper copies with him, however, he had taken a photo of them on his cellphone which the HSS confirmed had the Veteran's name, full SSN, DOB and medical information. He was informed that he should return the documents to the Privacy Officer (PO). He said he did not have the paper copies with him. He was provided with the PO's phone number.
