Issue:
Veteran A was given his Daily plan summary at check in at the CBOC. When the provider entered the room, Veteran A showed him that Veteran B's information was stapled to it. The Veteran gave it back to provider.
Outcome:
06/17/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
