Issue:
On 07/15/11 at 5:00 PM housekeeping employee found 3 folders on a table adjacent to the main entrance to the hospital. Housekeeper turned folder over to Admissions supervisor. Two folder contained pharmacy discrepancy reports, which includes inpatient name and medications, approximately 15 IN Patients. One folder contained 8 employment applications for a VA position, included full name, partial SSN, home address, home phone, personal cell phone number, employment and educational history. Update: 07/19/11:Fifteen Patients will receive a letter of notification and 8 applicants will receive letters offering credit protection services.NOTE: There were a total of 21 Mis-Handling incidents this reporting period. Because of repetition, the other 20 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
Appropriate action taken by HR and service line. Letters mailed 8-19-11. Privacy officer provided re-education to employee on use and security of documents.
