Issue:
Patient A reported to the Medical Center Director's Office that he had received Patient B's medication reconciliation summary report when being discharged from the Emergency Department on 12/31/12. He turned in the copy he received in error to the Director's Office staff which was then reported to the Privacy Officer. The report contained Patient A's full name, diagnoses and medications. No SSN or DOB was contained in the report. The Privacy Officer has reported the incident to the ED Nurse Manager who will investigate how the error occurred so the appropriate corrective action can be taken. Update: 01/22/13:Patient B will be sent a notification letter.
Outcome:
Nurse Manager educated the provider on the requirement to confirm the patient identifiers (name) before releasing the medication reconciliation report to the patient.
