Issue:
A VA clerk at the Peoria Community Based Outpatient Clinic (CBOC) mailed Veteran As lab information to an attorney rather than tp the physician that it was intended for. It appears that the clerk chose the incorrect name in the Release of Information (ROI) system. The party who received the information has returned it to VA Illiana Health Care System. Update: 11/02/11:Veteran A will be sent a letter offering credit protection services.
Outcome:
The employee will receive oral counseling along with some additional training. Credit monitoring letter sent to patient.
