Issue:
The switchboard operator received a phone call from a Veteran who received a signed form from his Primary Care Physician with another Veterans form included in the letter. The form was for requesting Handicap Access and contains Veteran's name and full SSN. The reporting Veteran is mailing the form back to the Huntington VAMC with attention to the Privacy Office for follow-up. Update: 08/24/12:Veteran B will be sent a letter offering credit protection services.
Outcome:
Staff reminded to be cautious when handling documents with PII.
