Issue:
Based on interview and record review, the hospital failed to ensure that the protected health information of 6 patient's was kept confidential from anyone not authorized access.Findings: An interview with the Compliance Manager (CM) was conducted on 11/21/12 at 10:05 A.M. The CM stated that RN 1 inadvertently mixed the nurse's worksheet, which contained the information of 6 different patients, with the discharge packet of Patient 1. According to the CM, the nurse's worksheets were used by nurses as a tool to inform them about their patients. The nurse's worksheet contained 6 patient names, dates of birth, medical record numbers, diagnoses, medical history, allergies, labs, diet, and code status.
