A local reporter stated in phone conversation to Public Affairs Officer (PAO) that he had an unredacted Office of Public and Intergovernmental Affairs (OPIA) fact-finding report and unredacted Emergency Medical Services (EMS) report. The Federal Health Care Center (FHCC) does not know who disclosed the reports. It has not been confirmed if the reporter actually has unredacted reports. More information as we receive it will be forthcoming.

Outcome:

04/20/15: The editor acknowledged they are in possession of the documents. A copy of a redacted fact-finding report was sent to the reporter in response to a Freedom of Information Act request but it is unknown how the media obtained the unredacted copy. The documents include information on one deceased Navy Veteran. 04/23/15: The Privacy Officer has reported that the Veteran has since become deceased.
