Issue:
A pharmacy resident placed a binder, containing a list of 32 inpatients (including full SSN, names, dates of birth and medications) in a backpack. The backpack was tossed in the trunk of his car. The resident went to dinner on Saturday night and used valet parking. When the car was returned, the resident noticed that several items had been stolen, including money, a bank document and the backpack. The incident was reported to the valet service and to the police. Update: 04/10/12:Thirty-two patients will be sent letters offering credit protection services due to full name and full SSN being stolen.
Outcome:
Pharmacy Resident met with his Supervisors immediately after incident occurred, after which time, he resigned. For new Pharmacy, Psychology and Psychiatry residents expected in July, they will be expected to attend New Employee Orientation, as is the current practice. In addition, the Privacy Officer will meet with them individually to explain the proper way to access, transport and dispose PHI.
