Issue:
A Postal employee returned four pages of documents from a Veteran to the VA . The documents were printed from the Release of Information department and mailed to the Veteran. The Veteran was contacted to report the event and asked if she had received any documents. She had just received a package from the USPS that indicated that the envelope was lost in the mail and opened. They returned to her pages that were mixed up and pages were missing. We are sending her a fresh complete package by Fed Ex. However, her information is unaccounted for. Update: 03/12/13:The Veteran will be sent a letter offering credit protection services.
Outcome:
Credit Monitoring letter sent to Veteran. Documents printed for Veteran again and sent via Fed Ex traceable delivery.
