Issue:
Release of Information Clerk mailed Patient B's lab results to Patient A. Lab results also contained Patient B's full name, full SSN, and date of Birth. Update: 11/13/12:Patient B will be sent a letter offering credit protection services due to full name and full SSN being disclosed.NOTE: There were a total of 23 Mis-Mailed incidents this reporting period. Because of repetition, the other 22 are not included in this report, but are included in the "Mis-Mailed Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
The Privacy Officer has notified the Release of Information Supervisor of this incident, and VA staff responsible for mailing patient information have been reminded that we are responsible for protecting the privacy of our patients information. VA staff were also instructed to take the appropriate precautions to prevent a recurrence of this type of incident.
