Issue:

A former VA Employee found documents when unpacking moving boxes. The documents found were described as "Master Record" pages. This is the second incident of found documents by the former employee. During the first incident the former employee indicated there may be other documents in boxes. The ex-wife of the former employee is a current VA employee. The Privacy Officer is arranging to obtain documents from the former employee. Update: 11/29/11:Five (5) Veterans will be sent a letter offering credit protection services due to full name and full SSN being out of VA control.1/17/12:There was an update from the PO. There are only three Veterans affected, one of them being deceased. Two letters offering credit protection services will be sent, and one next-of-kin notification.

Outcome:

The Privacy Officer will be conducting in-services with the nursing staff in conjunction with the Chief Nurses and the Nurse Managers regarding privacy related issues.
