Issue:

A provider reported to the Privacy Officer (PO) that he had taken video recordings of a patient, with the patients written consent. The provider then sent the video to a private-public educational medical company for educational purposes without the patients consent. Provider never heard back from the company and did not receive any indication that material was ever used. The provider recently realized he had made an inappropriate disclosure and reported incident to PO. The patient has passed away. Update: 01/20/12:The Veterans Next of Kin will be sent a notification letter.

Outcome:

Provider was required to retake Privacy training. Provider has increased his awareness by ongoing training and education.
