Issue:

A patient received billing information from his recent fee basis provider. However, the envelope arrived with a torn address window, and was originally addressed to the VAMC's billing department. Visible through the torn envelope was the Veteran's full SSN and home address, to which the US Postal Service delivered. The patient's address and full SSN were exposed through the tear in the envelope. The billing form appears to be a national form used by fee providers. The patient destroyed the envelope, but will submit a copy of the billing statement for further analysis. The envelope was in the custody of the USPS and was not delivered to anyone else. Update: 03/19/12:Two (2) Patients will receive letters offering credit protection services.

Outcome:

Credit monitoring letters offered to affected Veterans. Torn envelope attributed to USPS.
