Based on interview and record review, the hospital failed to ensure that Patient 1's medical information was kept confidential from anyone not authorized access.Findings:An interview with the hospital's Patient Relations (PR) was conducted on 5/10/12 at 3:20 P.M. According to the PR, both Patient 1 and Patient 2 were seen in the hospital's emergency department. The PR stated that Patient 1's discharge instructions and medication prescription were inadvertently given to Patient 2. The discharge instruction sheet indicated Patient 1's name, medication and diagnoses. The medication prescription indicated Patient 1's name, date of birth, date of service, and medical record number.
