Issue:
A number of checks and explanations of benefits for Fee payments were mismailed to an individual. The individual reported the issue and returned the checks without the original envelopes to Health Resource Center (HRC). HRC in turn advised our facility. It appears 4 individuals last name. FI, MI, and full SSN were disclosed and 7 individuals first and last name. Privacy will investigate. Update: 05/18/12:The four patients that had their full SSNs disclosed will be offered credit protection services. The seven others had full name, dates of exam, and a clinical code disclosed. The seven will be sent HIPAA notification letters.
Outcome:
Finance has updated the station standard operating procedure guide on Vendorizing submissions to incorporate address screening and to follow the Finance Service Center requests to properly vendorize. Staff members will be taking the following courses in TMS; VHA Privacy and HIPAA training as well as VA Privacy and Information Security Awareness and Rules of Behavior training. Credit monitoring letter mailed to the four affected Veterans and HIPAA notification letter mailed to the seven CHAMPVA beneficiaries.
