Issue:
Veteran A contacted the Privacy Officer (PO), Nashville Campus, regarding the following concern. Veteran A received a Patient Notification Letter in the mail which belonged to Veteran B. The letter was enclosed in Veteran As envelope. Veteran A incinerated Veteran Bs letter. Veteran Bs personally identifiable information (PII) included the full name, address, biopsy results, and providers name.
Outcome:
01/30/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
