Issue:
An employee's day planner was stolen/taken on 04/23/13. It happened around 8:15 AMor so. The employee was leaving the Muncie Community Based Outpatient Clinic (CBOC) and getting the GSA vehicle filled up with his stuff. He placed his planner on the roof of his personal vehicle parked next to the GSA vehicle. He was checking voice messages and then left for his destination. When he got to his meeting about 45 minutes later, he realized his planner was not with him. He called the CBOC and had his student go out to look on his personal car but the planer was gone. He had some notes on two possible Veterans who had never enrolled in VA. He also had a copy of a National Guard discharge for a Veteran. He was taking the discharge paper to the Veteran's VSO (Veteran Service Officer) that day to be placed in his claim folder. He filed a police report with the Muncie Police Department. No one has turned the planer or papers in to the CBOC yet. Update: 05/01/13:All three Veteran's information in the day planner included full SSN or date of birth. All will be offered credit protection services.
