Issue:
Based on interview and document review, the hospital failed to prevent unauthorized access to one patient's personal health information.Findings:In interview on 2/29/12 at 12 noon, Staff A stated that on 2/3/12, Staff B received a call from Physician C that he had received Patient 1's nuclear medicine study report. Physician C told Staff B that although he was listed as the admitting doctor, he had no record of ever treating Patient 1. Staff A stated Staff B determined that the registration clerk had transposed Physician C's name from the patient information above Patient 1 on the scheduling list. Staff A stated that Patient 1 and the Department were notified of the error on 2/8/12.Record review on 2/29/12 demonstrated that Patient 1's breached personal health information consisted of age, sex, medical record number, medical history and physical examination report, diagnosis, test results, treatment, plan, service date, and treating physician.Record review on 2/29/12 confirmed that the Department and Patient 1 were notified of the error on 2/8/12, within 5 business days of discovery of the error.
