Issue:

On 02/04/15, while at an appointment at the VA Chattanooga Outpatient Clinic, Veteran A was provided various documents upon checking out. When Veteran A got home and started reviewing the documents provided to him, he then noticed that he was provided two documents that belonged to Veteran B. On 02/05/15, Veteran A called and spoke with the Clinic Manager at the VA Chattanooga Clinic to report this incident. Veteran A agreed to return the documents of Veteran B to the Clinic Manager. Upon receipt of those documents, they will be provided to the Privacy Officer (PO) so it can be determined what personally identifiable information (PII) or protected health information (PHI) was involved.

Outcome:

02/10/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
