Issue:
Incorrect lab orders were given to the wrong Veteran. Correct information was faxed directly to the lab after event occurred. Incorrect orders were recovered. Unable to determine which Medical Services Assistant (MSA) gave the incorrect orders to the Veteran. Education regarding the importance of double checking the order before giving it to the Veteran will be discussed at the next staff meeting at the Bangor Community Based Outpatient Clinic (CBOC). Update: 12/15/11:One Veteran will be sent a letter offering credit protection services due to full name and SSN being disclosed.
