Issue:

Following a Veterans Medical Research Foundation (VMRF) study intervention, a Without Compensation (WOC) Research Assistant turned in the study data forms to the co-investigator, however he inadvertently took the study compensation form with him in his backpack. The backpack was left in his car overnight in front of a church nearby his residence. The next morning, he discovered that his car had been broken into and the backpack had been stolen. The Research Assistant filed a Police Report that day. The study compensation form contains the participant's name, address, phone numbers, questions about VMRF and VA employment status, signature, IRB study number, and amount paid. The form DOES NOT contain any study data or study ID numbers. Update: 11/03/11:There are three (3) Veterans in the study, each will be sent a letter offering credit protection services.

Outcome:

The person involved the incident has been being requested to go through the training again and the new process are being put into place. The PI submitted the an action plan to the PO with the steps they are taking and plan on taking with timelines , as well as proof when any training is completed. The PO also informed PI that they will have to notify the patients involved of what happened.
