Issue:

Patient A reported to medical staff at an outlying clinic that he had received another patients information with a prescription he received from the Grand Junction, CO VAMC Pharmacy. Patient A provided the staff member with the box the prescription came in which had a label with his name on one side and the name of Patient B on the other side. Patient A also provided the medical information sheet for the medication which had Patient Bs full name, address and the description of what the medication was used for. Update: 09/19/12:Patient B will receive a HIPAA letter of notification.

Outcome:

Remedial training to be provided to all Pharmacy staff. Notification letter sent to affected Veteran.
