Veteran A received Veteran B's fee basis authorization and consultation (Non-VA Care-Surgery-Plastics). Veteran A provided this information when a Patient Services Assistant contacted Veteran A on 05/01/12. In addition to his fee basis authorization and consultation, Veteran A received Veteran B's information in the mail on 04/28/12. Consultation Report included full name, address, date of birth and protected health information on Veteran B. Veteran A advised Patient Services Assistant that he will return the documents to her office. Update: 05/03/12:Veteran B will be sent a letter offering credit protection services.

Outcome:

Supervisor re-educated staff and stress the importance of safeguarding patient information.
