Issue:

Veteran A also received Veteran B's lab results in the same envelope as his own. Veteran A mailed the lab results to Veteran B, who subsequently notified telephone triage. The lab results contained Veteran B's full name, address, and CT results, which contained 6 sub-elements which were deemed "not satisfactory" for Veteran B. No SSN or DOB was listed on the letter. We are working with HAS to determine the method and means by which the test results were mailed, since the Provider is located at an Outpatient Clinic (OPC) and test results are often electronically routed through the main clinic mailroom for automated mailouts. Update: 08/09/12:Veteran B will be sent a letter of notification.

Outcome:

Mailroom clerks have been asked to "fan" stacks of test results before placing them in the hopper for automatic mail out, which consists of machine folding and stuffing each letter into an envelope.
