Issue:

Veteran/employee reports that he was brought to the VA after a suicide attempt for treatment on 1/3/15. On 1/4/15, a VA employee called his co-worker and asked if the co-worker knew about it and disclosed information about the event. The Veteran/employee states a lot of employees are now approaching him and asking how he is doing. He believes his privacy has been violated.

Outcome:

01/23/15: The ISO provided the PO with a SPAR. The PO requested the assistance of Service Mgt to determine if questionable accesses were in the function of assigned job duties. Service Mgt was able to confirm all but for 5 employees. Access was disabled and disusered to mitigate risk. Each employee was interviewed by the POs and ISO. All accesses were able to be confirmed to be in the function of assigned job duties by one. This employee admitted to accessing the chart out of curiosity, but insists there was no further verbal disclosure about the episode of care. So basically, thus far we have two improper verbal disclosures and an inappropriate access. This investigation is ongoing. We are awaiting a response regarding an access from a Tampa employee and an alleged verbal disclosure by a fee provider.
