Issue:
An enrollment clerk mistakenly enclosed Veteran B's DD214 and 10-10EZ application with a cover letter addressed to Veteran A instructing him to complete the attached 10-10EZ Form and return it to the Enrollment Center. Veteran A subsequently contacted and delivered the DD214 and 10-10EZ to Veteran B. Veteran B contacted the Enrollment Section by phone and next day showed the mis-mailed documents to the Enrollment Supervisor, who retained a file copy for follow-up. Veteran B was understanding of the situation and was later successfully registered in the system. The Enrollment Supervisor has traced the mistake to a VA employee. Further reporting as details warrant. Update: 09/02/11:Veteran B will be sent a letter offering credit protection services due to full name and full SSN beint disclosed.
Outcome:
Employee cautioned to be more diligent when preparing envelopes for mail outs, and to double-check the contents each time. This appears to have been a momentary lapse in attention. Credit monitoring letter provided to affected Veteran.
