Issue:
VA Employee A found two Hospital Inquiries (HINQ) run by VA Employee B on 02/15/12. VA Employee B admittedly left the HINQs in the restroom. The paperwork was found and turned into the Privacy Officer (PO) on the same day within approximately 4 hours of being left into the restroom. The HINQ contains the Veterans' name, address, date of birth, full SSN and disability code. Update: 02/16/12:Two Veterans will be sent a letter offering credit protection services.
Outcome:
Responsible service chief is taking disciplinary action. The Privacy Officer has also suggested the employee retake privacy training.
