Issue:

On 03/31/15, Veteran A contacted the Outpatient Pharmacy, Nashville Campus, and reported he received narcotic medication, via United Parcel Service (UPS), belonging to Veteran B. Veteran B was contacted and confirmed he received narcotic medication belonging to Veteran A. The Supervisor, Outpatient Pharmacy, Nashville Campus, informed Veterans A and B she would be sending them a UPS box (with a TVHS label) to utilize for returning the medications to the Outpatient Pharmacy, Nashville Campus. The Veterans providers were contacted and arrangements made to replace the medications. Upon investigation, the Supervisor, Outpatient Pharmacy, found the incident was due to a human error. The labels were inadvertently placed on the wrong Veterans package during the packing process in Outpatient Pharmacy, Nashville Campus. Veterans' personally identifiable information (PII) includes their full name, address, and name of medication.

Outcome:

04/01/15: The Incident Resolution Service Team has determined that Veteran A and B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
