Issue:

Veteran A was given lab results from the clinic staff at a Community Based Outpatient Clinic. That same day Veteran A realized that they were lab results of Veteran B also contained in the documents. Veteran A returned the documents pertaining to Veteran B to the CBOC Clinic staff the same day. Update: 06/14/12:Veteran B will be sent a letter offering credit protection services due to full name and SSN being disclosed.NOTE: There were a total of 26 Mis-Handling incidents this reporting period. Because of repetition, the other 25 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

The Clinic staff have been reeducated about proper quality assurance measures when providing documents to patients.
