Issue:

The Nurse Manager, Murfreesboro Campus, notified the Privacy Officer, Murfreesboro Campus, of the following incident: On October 1, 2013, Veteran A was treated at the ER, Murfreesboro Campus. On October 3, 2013, Veteran A had an appointment at the Chattanooga Outpatient Clinic (COPC) and was asked to provide the paperwork from his ER visit. Upon presentation of the paperwork, COPC staff noticed the documents included a medication reconciliation sheet which belonged to Veteran B. Veteran B had been treated at the ER, Murfreesboro Campus, on September 30, 2013 and was admitted to 7B later that evening or in the morning of October 1, 2013. Veterans B personal identifiable information includes: full name, full social security number, DOB, and medication list.

Outcome:

10/04/13: Veteran B will be sent a letter offering credit protection services, as his full SSN was released.
