Issue:

A preventive medicine note regarding Veteran A was inappropriately documented in Veteran B's electronic medical record. To compound the error Veteran B's full name, full SSN and DOB were disclosed to Veteran A when he was given and educational copy of the note. Update: 03/26/13:Veteran A and B will be sent a letter offering credit protection services.NOTE: There were a total of 30 Mis-Handling incidents this reporting period. Because of repetition, the other 29 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

Credit Monitoring letter has been sent to impacted Veteran, education has been provided to clinical nursing staff regarding the proper identification of Veteran/patient, prior to any procedures, charting or releasing of educational materials (medical records).
