Issue:

Veteran A received an appointment letter belonging to Veteran B. Veteran A stated he has received several pieces of mail for Veteran B but could not remember the contents and since the address (on the letter inside the envelope) was not far from his home he dropped it off. The Privacy Officer (PO) informed Veteran A not to go to the home but to instead bring or mail any documents to the PO instead. The appointment letter contained Veteran B's name, address, date of birth and partial SSN. Update: 04/23/12:Veteran B will be sent a letter offering credit protection services. His name and date of birth were disclosed inappropriately.NOTE: There were a total of 36 Mis-Mailed incidents this reporting period. Because of repetition, the other 35 are not included in this report, but are included in the "Mis-Mailed Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

Veteran B finally contacted the PO with his current address and the credit protection letter was sent today. The PO also advised Veteran B to come to the facility to Registration with his ID so his current address can be entered into the system. It is not known who mis-mailed the documents but all staff in the area have received guidance from supervisor on the need to be careful when mailing documents.
