Issue:

Based on interview and record review, the facility failed to prevent unauthorized access and disclosure of Patient 19's protected, confidential health information, when the patient's Emergency Room "charge sheet" was left in another patient's room. This failure allowed for potential unlawful or unauthorized use of the information and violation of the patient's right to privacy. Findings: During an interview on 11/11/14 at 11 a.m., the Privacy Official confirmed that the patients' information had been inadvertently left in another patient's room by a physician, when the physician was making rounds. The Privacy Official confirmed the information which had been disclosed in the document.Interview and review of the letter submitted by the facility revealed that the information included name, date of birth, address, medical record and account numbers, and diagnosis.The incident resulted from the physician's failure to keep private information secure.
