Issue:
Prescriptions for two patients were sent to a third patient by mistake. The two patients' names and medication information were compromised. Update: 03/21/13:Two patients will be sent a HIPAA notification letter.
Outcome:
Analysts continues to examine the pharmacy process to try to discover why this error occurred. It appears to be a glitch in the computer system.
