Issue:

Per the Dental Record Manager Plus a Veterans record was sent to Medical Records for retirement on 04/19/12. Dental requested Medical Records to retrieve the retired record on 10/09/12; however the Veterans CPRS was not annotated with a retired location as is normal procedure. The facility is unable to confirm the record was retired by shipment to NARA with the electronic record not properly annotated. Without the shipping information, NARA could not locate the Record. Privacy Officer, Dental, Medical Records and Logistics have searched for the record to no avail. Without confirmation of retirement, the facility must assume the record is lost. Update: 10/26/12:The Veteran will receive a letter offering credit protection services.

Outcome:

A thorough search was conducted to locate the record to no avail. The record may have been sent to NARA for retirement without being properly annotated in the Veteran's record, however this cannot be confirmed. Medical Records and Dental to create procedures to ensure records are retired correctly. Staff reminded of procedures when retiring records. Credit monitoring letter uploaded and mailed on October 25, 2012.
